Basically, CDEq "listens" to what the music is doing on the audio CD in your CD-ROM drive, and changes the picture on the CDEq window appropriately, either by altering the colour, the image itself, or both. The result is some very interesting shapes that alter in time with the music. You can choose the "image" that will be drawn in the CDEq window. (This is not an image in the sense of a bitmap, but a routine provided by a CDEq image library - "Image" was the best description). You can also change how CDEq listens to the music, and change some general options, such as automatic image changing, and what happens if the screen saver kicks in.
